Seventeen-year-old high school senior and cheerleader Megan Bloomfield is dating a handsome football player named Jared; however, she is reluctant to kiss him on the mouth and instead fantasizes about her fellow cheerleaders. Concerned about Megan's potential lesbianism, her parents Peter and Nancy, assisted by her friends, stage an intervention facilitated by ex-gay Mike, an employee of conversion therapy camp True Directions, which aims to convert attendees to heterosexuality through a two month five-step program in which they admit their homosexuality, rediscover their gender identity by performing stereotypically gender-associated tasks, find the root of their homosexuality, demystify the opposite sex, and simulate heterosexual intercourse. Despite denying her lesbianism, Megan is sent to True Directions.
Upon arrival, Megan meets strict disciplinarian Mary J. Brown, the program's director. While touring the camp with the nerdish Hilary Vandermueller, she meets and befriends college student Graham Eaton. Although more comfortable in her sexuality, Graham was forced to attend the camp or risk being disowned by her family after her stepmother caught her having sex with another girl. Megan is also introduced to several other adolescents and young adults attempting to cure themselves of their homosexuality: a biker goth Sinead Lauren, a retail worker Clayton Dunn, a varsity wrestler named Dolph, a softball player named Jan, a flamboyant actor and dancer named Andre, and a Jew named Joel Goldberg. The group's prompting forces her to reluctantly admit her lesbianism, which contradicts her traditional religious upbringing and distresses her, so she puts every effort into becoming heterosexual. Early on in her stay, she shockingly discovers Clayton and Dolph making out. After Mike catches them in the act, Mary dismisses Dolph from the premises and punishes Clayton with a week-long isolation in a doghouse.
Halfway through the program, Two of Mary's former students, ex-ex-gays Larry and Lloyd Morgan-Gordon, encourage the campers to rebel against her by taking them to a local gay bar called Cocksucker, where Graham and Megan's relationship becomes romantic. The next day, Jan quits the program after realizing she was never gay, to Mike's displeasure. Upon discovering what they did the night before, Mary summons their parents and schedules for them to meet with the campers individually, with Megan's parents threatening to disown her if she fails to follow through with the program. Mary soon lets Clayton out of the doghouse and later forces the campers to picket the Morgan-Gordons's house, carrying placards and shouting homophobic abuse. During the final test, Graham and Megan still gaze into each other's eyes. Andre is dismissed from the premises for failing the test while everyone else passes and prepares for step 5. Megan and Graham sneak away that night to have sex and begin to fall in love. When Mary discovers their escapade, Megan, now unapologetically comfortable with her sexuality, is dismissed from the premises. Graham, afraid that her continued defiance will result in her father potentially disinheriting her permanently, stays behind under the deal that she will partner with Mary's son Rock, who has been making multiple sexual overtures towards Mike and the other male campers, for step 5.
Disowned by her parents and homeless, Megan goes to stay with Larry and Lloyd, surprisingly discovering that Dolph now also lives with them. Meanwhile, Graham feels depressed without Megan, but obediently follows Mary's instructions. Megan and Dolph plan to rescue Graham and Clayton by infiltrating the graduation ceremony. While Dolph successfully coaxes Clayton away, Graham initially declines Megan's invitation to join them. Megan then performs a cheer she composed for Graham declaring her love for her, finally winning her over, and they drive off with Dolph and Clayton as they kiss. In the mid-credits scene, Peter and a greatly uncomfortable Nancy attend a PFLAG meeting to come to terms with their daughter's homosexuality.
